Bloodborne Pathogens Safety Training Document Protocol 

Kirk

  • Identifies jobs in which BBP training is required. He highlights them in yellow on the Labor Allocation Log.

  • Trains the foreman for each of the identified jobs. He leaves a training binder with them at the jobsite.

  • Notifies the foreman when a new hire is dispatched to the job that BBP training needs to be conducted on the first day.

 

 

Lead Forman

  • Conduct BBP training for all crew.

  • Conduct BBP training for all new hires on the first day they report to the jobsite.

  • Correct the Safety Quiz, review all incorrect answers with the crew member, and be available to answer any questions.

  • Collect all required paperwork and submit to Tucker

 

 

Tucker

  • Receives the required documentation

    • TM Safety Training Attendance Log

    • Bloodborne Pathogens Safety Quiz with Score

    • Vaccination Statement

    • Exposure Control Plan Safety Rules

  • Records the safety training in the License and Certification Log and files all documentation.

  • Forwards the Vaccination Statement to Ben

  • Records a “T” on the Labor Allocation Log

 

Ben

  • Receives the Vaccination Statement, records the responses on the BBP Vaccination and Reimbursement Log, and scans the statements to the BBP safety file.

  • Receives verification from employees that they have been vaccinated, records the date on the BBP Vaccination and Reimbursement Log, scans the documentation to the BBP safety file.

  • Releases Sue to reimburse when the entire vaccination series has been completed.

  • Double checks the designation of new jobs and training of new hires on the Labor Allocation Log.
